Overview
This flow describes how to fill a questionnaire with a saved response template. A template adds its medications and service requests to the form.
Pre-requisites
- You fill a questionnaire for a patient in a facility.
- You can see at least one response template. See Create a response template.
- You have the permission listed below.

Steps
1. Open the template list
Select Templates in the medications question or service request question. Care opens the Response Templates panel.
2. Find the template
Enter a name in Search templates. The list shows templates you created and templates shared with you or your departments.
3. Look at the content
Select the template. Care shows its medications and service requests.
4. Apply the template
Select Apply. Care adds all items to the form and closes the panel. To add one item only, select that medication or service request.
5. Change the values
Change the dose, schedule, or other details for this patient before submitting the questionnaire.
Expected Outcome
- Care adds the template items to the current questionnaire.
- You can change every value before submission.
